If you want to register with our GP practice, you can use the registration form.
To be registered as a patient with our GP practice, you have to reside in one of the following postal code areas (also known as a care area):
- 1234
- 3456
- 5678
- 7890
If you are already registered with a GP practice in this area, we request that you first contact us by telephone.
If you change of GP practice, you must inform your previous GP practice of your departure and submit a request to forward your file to our GP practice.
We are obliged to check your identity on your first visit to our GP practice, therefor we ask you to then bring a valid ID.
